Benefit Fact Sheet (Servicom-USA)
The following briefly outlines benefits available to employees. Additional information is available in your employee handbook. Procedures for taking advantage of these benefits can be obtained from vendor documentation (if applicable), your supervisor or Human Resources representative.
Medical Coverage
Upon completion of a 3 month introductory period, all employees are eligible to participate in the Company’s medical program through Cigna. A prescription drug plan is included as part of this program. Coverage is available for the individual employee plus his or her dependents/family. Employees must pay a portion of the insurance premium to secure and maintain coverage. Detailed information will be sent to employees shortly before their 3 month anniversary. Employees will also have the opportunity to enroll during the annual enrollment period.
Dental Coverage
Employees are eligible to sign up for Cigna dental coverage for themselves and their dependents at the conclusion of their 3 month introductory period. The cost of this coverage is also shared between the employee and the company. Participation in medical coverage is not required for an employee to obtain dental coverage. Detailed information will be sent to employees shortly before their 3 month anniversary. Employees will also have the opportunity to enroll during the annual enrollment period.
Vision Coverage (Available July 2006)
Employees who have completed 3 months of service are eligible to sign up for vision coverage through Spectera. Coverage is available for the employee plus his or her spouse and dependents. Premiums are paid by the employee and are very reasonable. Participation in medical coverage is not required for an employee to obtain dental coverage. Detailed information will be sent to employees shortly before their 3 month anniversary. Employees will also have the opportunity to enroll during the annual enrollment period
Short-Term/Long-Term Disability
The Company provides Short-term and Long-term Disability for employees that have completed their 3 month introductory period. There is no cost to the employee for this benefit however, they must complete an enrollment form to take advantage of it. If disabled and unable to work, an employee could receive up to 60% of his or her pay for up to a certain amount.
Life Insurance/Accidental Death & Dismemberment
The Company provides $20,000 in Life Insurance and AD&D coverage to employees that have completed their 3 month introductory period. There is no cost to the employee for this benefit however, they must complete an enrollment form to take advantage of it. Employees have the option of increasing the amount of their Life Insurance or AD&D coverage in $10,000 increments at a nominal cost.
Vacation
Beginning with their first full month of employment, the employee begins accruing vacation. Employees may borrow against unearned vacation however, if they leave the company prior to accruing all of the vacation they have taken, the unearned amount will be deducted from their last paycheck. Accrual rates are determined by an employee’s length of service. Vacation time must be taken during the calendar year in which it is earned.
Holidays
The Company observes 6 holidays throughout the year. They are New Year’s Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving, and Christmas. Holidays that fall on Saturday will be observed on the preceding Friday while those that fall on Sunday will be observed on the following Monday. Employees become eligible to receive holiday pay once they have reached their 3 month anniversary.
Floating Holidays
In addition to scheduled holidays, employees receive two (2) Floating Holidays per calendar year to be used at their discretion with the appropriate approval. Employees who complete their three (3) month introductory period by June 1st are eligible for two (2) Floating Holidays that year. Employees who complete their 3 month introductory period after June 1st but prior to October 1st are eligible for one (1) Floating Holiday in that year. Floating Holidays may not be carried over from year to year.
Sick Days
Employees can accrue up to 3 sick days per year providing they have completed their 3 month introductory period. Sick Days are added to an employee’s “bank” on the following dates: January 2nd, May 1st and September 1st. An employee may accumulate up to 30 Sick Days.
Direct Deposit
Employees have the option of having their paychecks automatically deposited into their bank accounts each pay period.
Credit Union (Available June 2006)
JNet Communications, LLC has arranged with Affinity Federal Credit Union to allow employees to become members. There are many benefits to joining a credit union which will be outlined in the member materials that will be distributed in early June.
401K (Available April 2006)
JNet Communications, LLC has contracted with Fidelity to establish a 401K plan for employees. This plan will allow employees to save for retirement by investing a portion of their pre-tax pay in a variety of funds. In addition, employees will experience an income tax savings by participating in the 401K.

Procedures for taking advantage of these benefits can be obtained from vendor documentation (if applicable), your supervisor or a Human Resources representative.
Medical/Dental/Vision Coverage
Upon completion of a 3 month introductory period, all employees are eligible to participate in the Company’s Medical/Dental/Vision program through Great West Life. A prescription drug plan is included as part of this plan. Coverage is available for the individual employee plus his or her dependents/spouse. Employees must pay a portion of the insurance premium to secure and maintain coverage. Detailed information will be sent to employees shortly before their 3 month anniversary. Once eligible, employees may enroll and/or make changes anytime throughout the year.
Long-Term Disability
The Company provides Long-term Disability benefits for employees that have completed their 3 month introductory period. An employee may apply for LTD benefits after 120 days of continuous disability. There is no cost to the employee for this benefit however they must complete an enrollment form to take advantage of it.
Life Insurance
The Company provides $20,000 in Life Insurance to employees that have completed their 3 month introductory period. In addition, the employee’s spouse is covered for $5,000 and children are covered for $2,500. There is no cost to the employee for this benefit.
Accidental Death and Dismemberment (AD&D)
The Company provides $20,000 in AD&D insurance to employees that have completed their 3 month introductory period. This coverage is reduced to $10,000 when the employee reaches age 65.
Optional Life Insurance
Any employee that completes the 3 month introductory period may sign up for additional life insurance in $10,000 increments up to a maximum of $200,000 for themselves and their spouse. The employee is solely responsible for paying the premium for this insurance.
Vacation
Employee begins accruing vacation immediately upon hire. The rate at which vacation is accrued is commensurate with years of service. Vacation time must be taken during the calendar year in which it is earned. Employees may only take earned vacation.
Holidays
The Company observes the following paid holidays in Canada – New Year’s Day, Good Friday, Canada Day, Labor Day, Christmas Day, Boxing Day and Remembrance Day throughout the year. In addition, Canada observes the following unpaid US holidays – Memorial Day, Independence Day, and Thanksgiving. Holidays that fall on Saturday will be observed on the preceding Friday while those that fall on Sunday will be observed on the following Monday. Employees are eligible for holiday pay as long as they have received pay for 15 out of the 30 days prior to the holiday.
Floating Holidays
In addition to scheduled holidays, employees receive one (1) Floating Holiday per calendar year to be used at their discretion with the appropriate approval. Employees who complete their 3 month introductory period by June 1st are eligible to take a Floating Holidays that year. Employees who complete their 3 month introductory period after June 1st are not eligible until the following year. Floating Holidays may not be carried over from year to year.
Sick Days
Employees who have completed the 3 month introductory period accrue one (1) Sick Day on each of the following dates: January 1, April 1, July 1 and October 1. A maximum of 30 days can be accumulated.
Leaves of Absence
Employees that meet the stated requirements are eligible to apply for a Leave of Absence for any of the following reasons: Pregnancy, Parental Leave, Military Leave, or Education. Reinstatement for each type of Leave varies and is governed by Provincial Law and Company policy.
